With conventional two-piece implants, the process usually takes 2–3 months for the lower jaw and 4–6 months for the upper jaw. The difference is due to the greater bone density in the lower jaw, which ensures faster integration.
In healthy patients with good bone quality and excellent primary stability of the implant, immediate or early loading is possible in some cases. Basal implants allow for full restoration in just 3–5 days, as they are anchored in compact bone, which allows for immediate loading.
It is important to note that implants must not be loaded prematurely during the healing process. Even with good initial stability, around the second to third week this stability temporarily decreases whilst the biological process of bone remodelling takes place.
Stages of osseointegration
The process of implant integration with the bone goes through several successive phases, each of which is important for the final success.
Primary stability (primary osseointegration)
Primary osseointegration begins immediately after the implant is placed. Within the first 24–48 hours, a blood clot forms around the implant, which is crucial for the healing process. Secondary osseointegration is the stage of strengthening and maturation of the bone tissue around the implant. It begins around 6–8 weeks after implantation and continues for 4–6 months. During this period, the newly formed bone gradually mineralises and becomes increasingly strong and dense.
Bone remodelling is a dynamic process in which old bone is resorbed and new, more mature and organised bone is formed in direct contact with the implant surface. Once this process is complete, the implant is fully osseointegrated and can safely withstand normal masticatory loads.
Stabilisation and functional adaptation phase
Once the main phases of osseointegration are complete, the bone continues to adapt to functional loads. This stage can last for months or even years. Regular check-ups reduce the risks of peri-implant disease and bone loss.

What determines successful osseointegration?
